To Sign up for Daycare , you must complete the following steps:
1) Schedule an interview - All interviews must be pre-scheduled. Please give us a call during business hours and we will set up a time that is convenient for you. Interviews and temperament testing will take approximately 1 to 2 hours.
2) Fill out an Application - Download an application HERE and a pet profile HERE. Please make sure to bring the completed forms along with you to the interview. Applications will also be available at our facility.
3) Obtain a copy of vaccination records - All dogs must have current vaccinations against Rabies, Bordatella, and DHLPP. Bordatella must be given every 6 months. Your dog will not be admitted into Sully’s Stay and Play until we have proof of vaccinations. If we do not have the vaccination records during the time of the interview the temperament testing will be rescheduled.
Prerequisites:
All dogs must be spayed or neutered.
All dogs must be friendly towards other dogs and people. They must not be food or toy protective.
All dogs must be in general good health and free of any contagious diseases which could potentially jeopardize other guests.
